IMMUNIZATION REQUIREMENTS FOR ALL OBSERVERS MAKING TRIF TO ENIWETOK
PROVING
GROUND
Fach individual is required to possess an immmnization record
indicating that the follewlng immmnizetions have been accomplished:

dL,: Smallpox -- within the last three years.
2.
Typhoid -- within the last three years.
3.
Tetanus ~- initial series of injections or stimulating dose
within the last four years.
Civilians mst have in their possession Public Health Service
Form 731 (PHS-731) "International Certificate of Innoculation and
Vaccination.'

IF any participant has none of these immunizations:
1.
The smallpox immunization mist be accomplished before arrival
2,
Three typhoid injections are required, spaced at least one
week apart,
3.
Three tetanus injections are required and they take from three
in Hawaii, tentatively scheduled for April 23, 1956.

NCTE: The injections required under 2 and 3 can be completed at
Hawaii
and at the Enjwetok Proving Ground.
It is necessary to have PHS--731 completed and available for
presentation to the proper authorities upon return to Hawaii.
A fect sheet giving pertinent data on clothing, communications,
tentative schedule, mid other information will be furnished each principal
anc alternate upon receipt of certification of their nominations by the
designating organization,
